If a patient wants to undergo valve replacement surgery under the Mukhyamantri Ayushman Arogya Yojana at an approved hospital, it is mandatory for them to inform the health guide at the Mukhyamantri Ayushman Arogya counter about their Jan Aadhar card and Aadhar card on the same day.
The disease/doctor for whom to treat under Mukhyamantri Ayushman Arogya Yojana must be in a hospital linked to Mukhyamantri Ayushman Arogya Yojana.
It is mandatory to have a doctor's prescription which includes:
Diagnosis of the patient's disease by the doctor
The patient's complaint history and any previous diseases should be clearly documented
Details of the surgery to be performed as well as details of the implant should be provided
It is also mandatory to have the Chiranjeevi's code and the doctor's seal (RMC No.) and signature.
For valve replacement surgery, tests (such as angiography, 2D echo, and ECG) must be undergone as prescribed by the doctor according to the patient's medical history. Treatment in Mukhyamantri Ayushman Arogya Yojana is not possible without tests.
The patient's name must match across Jan Aadhaar, Aadhaar, and medical documents. If there is a mismatch, the patient must submit an affidavit.
The benefit of Mukhyamantri Ayushman Arogya Yojana is not payable only if CAG (angiography) is done.
Application Process:
First of all, the documents related to the disease are seen by the patient's hospital. It is mandatory to have the doctor's seal and signature and the doctor's registration number (RMC) written on the complete doctor's prescription and tests related to the disease.
Treatments for different diseases are given under Ayushman Arogya Yojana. Before admission, the patient must be tested according to the disease. They must also have their film along with Echo and other radiology reports.

Under Ayushman Arogya Yojana, the patient's photo and biometrics are processed during admission. In this, the hospital further sends the patient's documents to Chiranjeevi through the portal. If the patient's condition is standard on the portal, the patient is given treatment only after getting pre-approval (no waiting for approval in case of emergency). The time taken to get approval is approximately 2 to 4 hours, a maximum of 12 hours. This entire process of admission takes about 15 to 20 minutes for a patient.
At the time of discharge, documents are prepared by the hospital and uploaded to the portal along with the live photo of the patient and the photo of the patient along with the doctor. Also, the patient's feedback form is filled by the patient and uploaded on the portal.

Don’ts
A cashless facility is not provided in OPD under Chiranjeevi.
Treatment under Chiranjeevi Yojana is only possible with complete tests and a doctor's prescription.
